Notice of Privacy Practices for Fixsie Health

This Notice of Privacy Practices (“Notice”) took effect on December 19, 2024, and will remain in effect until We replace it. We must abide by the terms of this Notice while it is in effect.

Use of Your PHI

The Company may use PHI for the following purposes:

Retention of Your PHI

The Company will retain Your PHI only for as long as is necessary for the purposes set out in this Privacy Policy. Delete Your PHI

Business Transactions
